Community-Based Living Services - Evansville, Vincennes and Terre Haute, Indiana
State Waiver & Other Programs for Individuals with Disabilities

Bridges of Indiana is a provider for the State of Indiana Medicaid Waiver Program. These waivers allow bridges of Indiana to provide Residential & Community assistance to Consumers. We are certified to provide services under the following waivers:

Family Settings

This type of service is for anyone living in a family home and is not in need of round the clock care. Bridges of Indiana will provide services in their family’s home and in the community as seems appropriate. This service assists individuals in being independent, and it allows families with children with disabilities the independence and support they need.

Independent/Supported Living Services

This type of service is for the individual living in an independent setting. This service can be provided in a variety of settings including people’s own homes or supported living arrangements. This service assists individuals in being independent, while reaching all the goals and aspirations that they might have for themselves.

Respite Services

This type of service is to allow relief to the family member who is the caregiver of the Consumer. This service cannot be providing while family members are at work.

Attendant Care for the Aged & Disabled

This type of service involves "hands on" assistance for physical dependency needs of older adults and is furnished in the person’s home in order to allow that person to remain in their own home. Activities include incidental homemaker activities, bathing, grooming, meal planning, prep and clean up, escorting to medical appointments and assistance with correspondence and bill paying. Some funding sources are available; examples are CHOICE, Transitional or Diversional Waiver State Grants and the Aged and Disabled Waiver.

Employment Services

This type of service provides individualized service and supports for people with disabilities to prepare for, obtain and secure permanent paid employment.  It also provides suitable skilled workers to employers in the public workforce.  It is funded through The Office of Vocational Rehabilitation and is at no cost to the employee or employer.
